Investigation Summary
At approximately 11:30 a.m. on May 31, 2011, Employee # 1 was working on the roof of a remodeled commercial structure. The roof measure 13 feet 7 inches high above the ground. Employee # 1 and Employee # 2 continued working on the roof after completed work around the perimeter of the roof. A sheet of plywood (4 feet by 8 feet) was used as a temporary tools storage area. Employee #1 stated that the sheet of plywood was secured to the roof joists, prior to the accident. Employee #1 did not realize that the sheet of plywood was unscrewed before stepping at the edge of it, which caused Employee #1 to trip and fall. Employee #1 fell from the roof to the concrete floor. Employee # 1 was transported to Harbor UCLA Hospital, where he was hospitalized and treated for his injuries. Employee # 1 suffered fracture right ribs and two right fingers. No one witnessed the accident. The accident was reported to the Division (of Occupational Safety and Health) on June 1, 2011 by the company owner.
