Window Washer Is Killed in Fall to Ground

At approximately 10:00 a.m. on June 20, 2011, Employee #1, a 52-year-old male with Chamberlin Building Services Inc., window washing contractor, was on a roof preparing to descend from the parapet wall of a three story building to clean. A coworker, who was working near Employee #1 on the roof, heard something snap and he ran toward the falling employee and tried to catch the unwound rope. Employee #1 fell down to the ground and landed on the side walk. Employee #1 died from multiple internal injuries as a result of the fall. During the inspection, Employee #1's harness and rope grab device were found on the roof. Contributing factors to this event, Employee #1 was not tied off to the safety line or working line and was not tied to an approved anchorage. The employer was a window washing contractor, who uses swing stages, control descent apparatus and tucker poles to clean windows on buildings.
