Investigation Summary
On March 2, 2012 Employee #1 from Cal Coast Stucco Inc., got injured. Employee #1 was working with two other employees dismantling a metal scaffold. The scaffold had four levels and about 30 frames. At the time of the accident the three employees had dismantled several frames. Employee #1 was located on the third level of the scaffold, Employee #2 was located on the fourth level, and Employee #3 was located on the ground. Employee #2 was handing down a piece of the scaffold(an end frame), when Employee #1 fell. Employee #2 asked him if he "had it"(meaning the piece of scaffold) before he let go of it, and Employee #1 indicated "yes". A few seconds after Employee #2 let go of the piece he heard Employee #1 yelling "watch out, watch out". When Employee #2 turned around he saw Employee #1 falling from the beam of the building. Employee #1 fell approximately 11ft. to the concrete ground. The other employees immediately came down from the scaffold. Employee #1 was lying on his side. He asked the injured employee if he was alright, and he did not answer. He saw the injured employee was lying on top of his left arm, so he moved him a little to take his body off of his arm. He then went to the construction office and told them to call 911. A lady passing by the site had also called 911. Employee #1 received a broken left hand and wrist, several broken ribs on the left side and a fractured pelvis. Employee #1 was hospitalized at Henry Mayo Hospital, Valencia, for six days and had surgery on his left hand and wrist with a steel plate inserted
