Investigation Summary
On January 13, 2012, Employee #1 was assisting a manager and an outside repair person. The manager and the repair person had used a wood cleat ladder to go up to the elevated area, so they could look at the roof to ascertain where the leaks were coming into the building. Employee #1 was standing below on the floor when the manager asked him to get a flashlight and bring it up to him. Employee #1 got the flashlight, put it in his pants pocket, and proceeded to climb the ladder. When Employee #1 reached the top rung, he placed his left foot onto the landing, and was about to place his right foot on the landing, when the ladder began to slide down, causing him to fall 8ft to the concrete floor. Paramedics were called and Employee #1 was transported by ambulance to the hospital. Employee #1 suffered injuries to both hands and a laceration on his forehead. His right hand has two broken bones behind the ring finger and little finger, and the left hand has three dislocated fingers. Employee #1 was hospitalized for four days at a local medical center, and had surgery on his right hand with a metal plate and screws inserted. He also had surgery on his left hand to repair the tendons in the ring finger.
