Investigation Summary
At approximately 11:15 a.m. on February 10, 2012, Employee #1 was performing his daily duties for his employer, Safe and Beautiful Tree, Inc., a tree trimming and landscaping company. Employee #1 had worked for that company for approximately seven years, and he performed tree trimmer duties. Employee #1 was trimming an Italian pine tree. A limb he was cutting was rigged to a branch, and the branch broke, due to weight of the limb being cut. The branch struck Employee #1, and he sustained a concussion. Employee #1 was transported to Providence Holy Cross Medical Center at Mission Hills. He was hospitalized and treated for 12 days. Safe and Beautiful Tree, Inc., notified the Division about the accident at 10:06 a.m. on February 13, 2012. Employee #1 had been working adjacent to other employees, and a supervisor was on site. Employee #1's supervisor and coworkers were interviewed. The investigation concluded that no Title 8 standards had been violated in connection with the accident. As a result, the Division did not issue a serious accident-related citation. Apparently, only non-accident-related citations were issued.
