Investigation Summary
On September 14, 2011, Employee #1 and the foreman, Employee #2 were doing sheet metal work for their employer, a sheet metal company. Employees #1 and #2 were working on a scaffold that had been erected on the exterior of a building under construction, and they were assigned the task of applying a roll of moisture barrier material to the eaves of the roof. The top half of the building had been stuccoed two days earlier by another employer. The plastering company removed the ties connecting the scaffold to the exterior of the building in order to apply the color coat to the stucco, and did not notify the other employers of the missing ties. The sheet metal company did not conduct their own safety inspection on the scaffold prior to starting work. Both Employee #1 and #2 each placed an A-frame ladder on the scaffold, and leaned it against the eaves of the roof. One of the employees overreached to one side and shifted his weight, which caused the scaffold to tilt. Employee #1 fell 15-20 feet to the ground below and suffered unspecified fractures.
