Employee Struck by Falling Object, Later Dies

At approximately 2:30 p.m. on August 8, 2011, Employee #1 and two coworkers were assembling a large LED screen for use inside an arena. The large LED screen consisted of 20 panels, which each had dimensions of 30-in. by 40-in. and weighed approximately 100 pounds. In the parking lot, the employees locked five panels next to each, forming one row of the large screen. Next, one of the employees used a forklift (Model CAT TL943 Telehandler) to elevate the completed row, so they could assemble another row. After completing three rows of the LED screen, they began working on the fourth and final row. One employee was directing the forklift operator to lower the three rows onto the fourth row, which was sitting on the ground. As the partially completed screen, which had an approximate weight of 1800 pounds and dimensions of 17-ft by 10-ft, was placed upon the fourth row, the screen came off the forks. The screen fell and struck Employee #1, who was standing behind the elevated screen. He sustained crushing injuries. Two coworkers were unable to lift the LED screen from upon him. They called for help and five bystanders assisted them with lifting the screen and extracting Employee #1 from under the fallen screen. Employee #1 was taken to a medical center, where he was pronounced dead.
