Investigation Summary
At approximately 2:10 p.m. on July 26, 2011, an employee working as an apartment manager for the owner of Sepulveda Associates was working alone with an unguarded table Ski saw model# 3400. The employee was cutting a small piece from the end of a laminate bar top which was approximately 72 ft. long and 1 ft. wide by 1 in. thick. As the employee finished the cut, she lost her balance. She was holding the piece that was cut with her right hand and could not hold the rest of the bar top with her left hand, as she lost her balance; her right hand thumb came into contact with the saw blade resulting in an amputation.
