Investigation Summary
At approximately 7:00 a.m. on June 29, 2011, Employee #1 was working at a retail grocery supermarket. A customer in the meat department requested a certain cut of pork ribs. Employee #1 did not adjust the meat saw guard while cutting the pork meat ribs and the bone became stuck on the blade. Employee #1 pushed the pork rib meat with his right hand and his right hand slipped which resulted in the saw blade cutting the right middle finger of his hand. Employee #1 was hospitalized were surgery amputated his middle right finger with some bone loss.
