Investigation Summary
At approximately 1:00 p.m. on May 24, 2011, Employee #1 was assisting a coworker, the foreman, with the placement of an expansion joint between a scrubber and exhaust system ducting. The coworker left the area to obtain additional tools from the company vehicle. The vehicle could not be seen from the work area. Employee #1 obtained a 30 ft metal extension ladder from another contractor in the area, and leaned the ladder against the scrubber. He then proceeded to ascend the ladder. The footing of the ladder slipped on the surface and both he and the ladder fell to the ground. Employee #1 fell approximately 17.5 ft and sustained a left forearm fracture. He was transported to a medical center, where he underwent treatment and was then hospitalized. The employer did not instruct Employee #1 to proceed with the work while the foreman was away from the work area. The causal factor was that Employee #1 had not been trained on the proper use of extension ladders.
