Investigation Summary
At approximately 10:30 a.m. on May 29, 2011, an employee, a maintenance mechanic, was performing a regularly assigned task of servicing conveyor equipment for the purpose of installing rail guides along the path of belt. The employee and a coworker, another maintenance mechanic, working as a two-person team entered Building Number 51 for the purpose of installing rail guide on one side of a section of roller conveyor. The roller conveyor was running at time of entrance of both workers. The coworker cut a section of metal apart from the conveyor, and then left the area to obtain additional material. The employee proceeded to drill holes one above the other in sidewall of conveyor frame using battery-operated drill. The employee intended to place threaded bolt through hole after first inserting bolt through rail guide through wall, then fasten with a metal nut on other side of frame wall. After drilling both holes, the employee pushed a bolt through both openings. He removed the upper bolt first. The second bolt would not come out. The employee then used left hand to put push and tap the end of bolt by putting hand under the roller conveyor next to the moving belt and pulley assembly. The employee's left hand fingers, middle and ring fingers, were caught in the nip point and severed at the distal joint. The employee called for help and ran to meet the coworker at which time a call was made for the security department which called for medical assistance. The employee was taken by air ambulance to Kern Medical Center in Bakersfield where he received treatment to left hand and was discharged. The following day he was seen by the company's consulting physician who then referred him to San Joaquin Community Hospital in Bakersfield where complete amputation of two fingers was performed
