Investigation Summary
At approximately 2:30 p.m. on April 15, 2011, Employee #1, a construction laborer, was clearing concrete out of a ditch at a job site. The employer specialized in general cleaning activities that included: knocking down fences, pressure washing, and hauling concrete. While a coworker operated a Bobcat with a bucket attached, Employee #1 worked to load an approximate 200-pound concrete piece into the bucket. Employee #1 was located on a ten-degree slope. As he loaded the concrete piece into the bucket, Employee #1 slipped. This caused the large concrete piece to fall onto Employee #1's left leg, fracturing his left knee. Employee #1 was taken to a nearby medical facility, where he was hospitalized and treated for his injury.
