Investigation Summary
At approximately 10:07 a.m. on March 29, 2011, Employee #1 was performing a regularly assigned task of cleaning conveyor belts and floors of excess pistachios to prevent buildup and conduct cleaning maintenance. She had been cleaning conveyor tables of pistachio debris and sweeping the floor around and below them. Employee #1 knelt down beside the pistachio sorter table and stretched her right gloved hand under the table to clear the area of pistachios and dust. Her right thumb went between the in-running nip point of a chain and sprocket assembly that powered the conveyor belt. As the assembly was turning, Employee #1's gloved thumb was caught in a nip point, amputating half of her distal thumb phalanx. Employee #1 was transported to a hospital, where she received medical care, after which she was released with a referral to see a hand surgeon.
