Investigation Summary
At approximately 3:00 p.m. on December 23, 2012, employees were framing an RV garage on a cement slab behind a residence. The temporary truss support plate was to support the center of the trusses as they were set on the frame of the garage. The trusses were 40 ft. long and 7 ft. high, weighing approximately 100lbs each. Employee #1, a framer, was on top of the truss support plate. Employee #2 was on the ground attaching the trusses to a crane with metal wire slings and safety hooks. Employee #2 guided the trusses from the ground over the frame of the garage using a tag line. Employee #1 detached the two slings after the trusses were laid down. After several of the trusses had been placed on the support, it collapsed. Employee #1 fell approximately 15 ft. to the cement slab and fractured his head. The truss support plate was constructed of one 2x8 that was laid flat, but it was not secured lineally to a 2x6 plank laid on edge as required. Employee #1 was taken by ambulance to Kern Medical Center. The accident was reported to the Fresno District office by the Kern County Fire Department at 4:32 p.m. on December 23, 2010. The employer notified the Fresno office of the accident on December 24, 2010.
