Investigation Summary
At approximately 11:50 a.m. on November 3, 2010, Employee #1 was attempting to remove a short key from a shoe die. He was using a 2-in. wide by 3-in. high by 6-in. long steel block extension tool and key driver-suspended hammer. He was trying to drive the key until it become loose. The extension tool was balanced on a piece of 4-in. by 4-in. by 15-in. piece of wood when it was hit by the key driver-suspended hammer. The extension tool flew and struck Employee #1 in the face causing facial injuries, a fractured jaw and nose, and lacerations. Employee #1 was hospitalized.
