Investigation Summary
At approximately 8:00 p.m. on June 19, 2010, Employee #1 was injured while operating on a powered pallet jack. The accident occurred in the battery changing station along the east side of the building. At the time of the accident Employee #1 was riding a battery powered pallet jack. Employee #1 turned to allow a coworker more passing room, and in an attempt to avoid hitting a barrier, dismounted the pallet jack and broke his ankle. Employee #1 sustained a fractured ankle, as it was crushed between the barrier and the side of the pallet jack.
