Employee Slips and Gets Run over by Truck

On January 19, 2011, Employee #1 was one of four employees pushing a disabled 26,000 pound truck from a service bay to an outdoor storage location by hand. Employee #1 was pushing along the left side of the truck in front of the rear wheels. Employee #1 slipped and fell, and was run over by the truck's rear wheels, suffering fatal injuries.
