Employee Amputates Finger While Trying to Remove Slip Pin

At approximately 9:00 a.m. on January 27, 2011 an employee was attempting to remove a slip pin from the bottom of a ladder rack that was attached to the back of a flatbed truck. The warehouse manager was assisting the employee. The manager decided to get a screw driver and a hammer to remove a stuck slip pin. As the manager was getting the screw driver and hammer, the employee managed to get the pin a little loose and decided to stick his right hand middle finger in the hole to push the pin out, but the rack came down on his finger amputating the tip of his finger from the finger nail down. The injured employee was transported to O'Connor Hospital. The employer reported the accident to Cal/OSHA on January 27, 2011 via phone. Investigation into the accident began on February 8, 2011 at approximately 11:00 am. Opening conference was held with the owner of the company escorting the Division personnel through the warehouse and to the area where the accident occurred (In parking lot where truck was parked), showing photographs and providing measurements that were taken. At the time of this investigation, the scene had been changed.
