Investigation Summary
At approximately 2:10 p.m. on November 3, 2010, Employee #1, a truck driver, climbed up a Safe Track platform to open hatch doors on his tanker trailer. The Safe Track platform is surrounded with a guard rail and is approximately 12 ft high. Employee #1 was found on the ground and was hospitalized. He is not able to talk about what occurred and is in a coma.
