Employee Amputates Finger in Moving Part on Forklift

At approximately 1:13 p.m. on July 9, 2010, Employee #1 was loading a bag of starch on a Nissan 40 forklift via a push-pull b style attachment. To load the bag properly, he stood at the front of the attachment and attempted to straighten the slip sheet, which was located under the starch bag. A coworker was standing next to the controls and accidentally struck the control, which caused the clamping mechanism on the push-pull attachment to move down. The attachment struck and amputated the right little finger of Employee #1. Emergency medical personnel were summoned and transported him to a medical center where he received treatment and was hospitalized.
