Investigation Summary

Investigation Nr: 200556751
Event: 11/09/2011
Employee Is Struck by Forklift, Later Dies

On November 9, 2011, Employee #1, a mechanic, was removing the hydraulic hose from the equipment. His coworker was operating the fork lift with the mast fully extended when the hydraulic hose broke. Employee #1 was underneath the mast while it was up, and he tried to release the pressure, causing the load apron and forks to drop. The forks struck the coworker on the head and crushed Employee #1. The coworker was able to go look for help and called emergency services. The paramedics took both employees to the Teche Regional Hospital in Morgan City, where the coworker was treated and released. Employee #1 was admitted to the hospital for two days, but was transported on the third day to the Oshner Medical Center in New Orleans. Employee #1 died on November 11, 2011 due to internal injuries.
