Event: 09/03/2010
Employee Dies from Cerebral Infarction While Welding

On September 3, 2010, Employee #1, a welder, was cutting and removing a section of the bow rake bulkhead, located inside of the barge behind the bow rake void on the port side. This was being done in preparation for replacing the section of the bulkhead with a new piece of metal. At approximately 10:45 a.m., Employee #1 was found collapsed on the interior bottom surface of the hull. Employee #1 was the only employee working in the compartment at the time and had been working alone for three to four days. According to the autopsy, the cause of death was cerebral infarction, which can cause seizures and loss of consciousness. It was noted that this had also occurred three months earlier.
