Employee Dies Falling from Scaffold

At approximately 2:30 p.m. on July 22, 2010, Employee #1, a laborer, was standing on a scaffold on the exterior of the building and engaged in demolishing a concrete block wall on the north west side of the building. Two coworkers, were also engaged in demolishing the concrete block wall of the building at the time of the incident. Coworker #1, was on the second level of the scaffold on the exterior of the building assisting in removing the demolished concrete block debris. Coworker #2, was on the interior of the building, demolishing the concrete block wall of the building, by breaking the joints between the concrete blocks with a hammer drill. Employee #1 walked over to a water cooler that was on the same level, on which work was being performed. The scaffold that Employee #1 was standing on did not have appropriate guardrails installed in order to provide adequate fall protection. Employee #1 fell off of the second level of scaffold, and landed on the concrete slab below. Employee #1 suffered head injuries, a broken pelvis and broken ribs from the fall, Employee #1 died from sustained injuries.
