Investigation Summary
At approximately 12:00 p.m. on August 12, 2012, Employee #1 with Cal Pacific Export Packers Inc. was picking up wood near the Plywood Area while a coworker was driving a load of lumber with the Hyster 50 Forklift near the Plywood Area. Employee #1 saw the coworker backing up the Hyster 50 Forklift towards him, and Employee #1 yelled at the coworker to stop. The coworker did not stop and struck Employee #1, crushing the two metacarpal bones on the right foot. Employee #1, was taken to the hospital, where he was diagnosed with a fracture to his right foot and released that same day. However, Employee #1 subsequently developed Osteomyelitis in the Right Foot. On October 17, 2012, Employee #1 was admitted to another hospital, where he underwent surgery and was hospitalized in excess of 24 hours.
