Investigation Summary
At approximately 6:46 p.m. on November 19, 2012, Employee #1 was walking while a coworker was repositioning a pallet of stock that had been double-stacked and was operating a forklift (Crown Lift Truck, #057). The coworker was not able to visually verify that the forks were correctly inserted into the slots of the pallet. When the coworker made the lift, the top pallet and its contents fell over into the aisle where Employee #1 had stopped to pick up some trash that was on the floor. The falling pallet and its contents struck the employee. A coworker called the Paramedics and Employee #1 was transported to Riverside Community Hospital, where he was treated for a head concussion and facial fractures. Employee #1 remains hospitalized.
