Investigation Summary
At approximately 12:30 p.m. on May 30, 2012 Employee #1 of ETO Doors, Inc. was seriously injured. Employee #1 began his normal job of cutting doors with a panel saw in the Free Hanging Department. He looked around for more work and observed a large stack of door jams. He then began cutting the door jams on a SLM Table Saw (Model SC3 S/N XK/005520). Employee #1's left thumb was amputated when he reached over the unguarded table saw blade because the door jam kicked back. This was not Employee #1's normal job and there were no witnesses.
