On January 26, 2012, Employee#1, baker, Maplegrove Gluten Free Foods, lnc., fractured/lacerated finger on left hand clearing a pasta jam on a spaghetti stripper (Make: De Francisci; model: Costa Macaroni; Number 01138). Employee#1 hit stripper dislodging jam, caught hand in unguarded gears/sprockets. The stripper was not locked (no tag out); power was on. Employee #1 was treated at hospital.
