Employee Seriously Injured in Chopping Machine

At approximately 9:00 a.m. on August 16, 2011, Employee #1 sustained serious injuries while operating a meat and vegetable chopper at local grocery store. Employee #1 loaded leeks from containers onto the lower conveyor belt on the meat and vegetable chopper. The leeks kept building up near the tail pulleys of the upper and lower conveyor belts, so Employee #1 would push the leeks with her hand to help move it through to the chopping blade. At one point when Employee #1 pushed the leeks, her hand contacted both conveyor belts near the tail pulleys, and drew Employee #1's hand toward the rotating chopping blade. As the conveyor belts drew Employee #1's hand inward, the tail pulley on the upper conveyor began to rise upward increasing the opening to the point of operation and allowing her hand to contact the rotating blade. Employee #1 sustained injuries severe enough to warrant a stay at a local hospital.
