Investigation Summary
At approximately 1:30 p.m. on August 24, 2011, an employee was working as a machine operator. His direct supervisor but was in the office when the accident occurred. The employee was working in shipping and receiving on a Dewalt radial arm saw, no model or serial numbers visible, cutting wood. He was cross cutting a nine foot piece of wood into pieces. As he was cutting the wood, it kicked back, pulling his left hand into the moving blade, resulting in the amputation of his thumb, index, and middle fingers. The employee was treated but was not hospitalized.
