Investigation Summary
At approximately 10:00 a.m. on July 1, 2011, Employee #1 was working in a cold storage warehouse facility. He was operating a forklift, ID DR-13, Model Number EASI DR30TT-A, Serial Number DZ-C-04-32346. He was picking orders from aisles that were to be loaded onto trucks. He was in Building Number 2, Freezer Room 10, traveling west from aisle 10-1 to 10-3. A coworker was traveling with an empty pallet with the forks forward and raised on aisle 10-2 on a Raymond reach truck, ID DR-3, Model Number EASI DR30TT-A, Serial Number EZ-B-02-23368. Employee #1 stopped at the end of the aisle, and the coworker turned right onto the cross aisle and collided with Employee #1. Employee #1 was in the operator compartment and was either struck with the fork or a piece of pallet in the lower right leg. Employee #1 was hospitalized with two fractured bones in his right leg. He also had ripped ligaments, veins, tendons, and nerves in his right ankle.
