Investigation Summary

Investigation Nr: 201693934
Event: 02/10/2011
Employee Fractures Back When Struck by Falling Wood Panel

At approximately 8:00 a.m. on February 10, 2011, an employee, a machine operator, was working in the Panel Saw Department. A coworker directed the employee to load the wood panels. This required that the load of material, the wood panels, be pushed and pulled over the metal rails on the ground. The employee loaded pieces of wood material in rolling cart. There were eight 24 by 8 panels and one smaller panel of uncertain size. There were rails on the ground for the movement of the carts. The employee was trying to pull the cart over the rails. The material in the cart fell and flipped over on top of the employee. The coworker was about 10 ft away from the employee, when the injury occurred. The height from the floor to the top of the cart measured 57-in.; the width of the cart was 20.5-in. and the height of the panels measured 88.5-in.. The factor leading to the accident was failure to secure the wood panels to the cart to prevent the panels from falling and causing injury. The employee was hospitalized and treated for fractured back.
