Employee Injures Back When Struck by Falling Auger

At approximately 6:50 a.m. on January 13, 2011, Employee #1 and two coworkers were assigned to continue work on the decontamination and demobilization of a centrifuge unit. This task involved the removal and cleaning of an auger. During removal of the auger, there was difficulty in removing a securing bolt that was located at the base of the auger. One of the coworkers applied WD-40 to assist in loosening of the bolt and made a second attempt to remove the bolt. However, the auger shifted. With Employee #1 standing by, the coworker stopped removal of the bolt came off the rig. Soon after leaving the rig, the coworker heard a popping noise. He turned towards the rig and saw Employee #1 on the ground injured with the auger beside him. Employee #1 was taken to a nearby medical facility, where he was hospitalized and treated for blunt force trauma his back. The accident investigation determined that the employer had no procedures in place nor were the employees appropriately trained for removal of the auger; the auger should have been secured before attempting to remove it.
