Investigation Summary
On August 23, 2010, Employee #1 and a coworker were connecting a 36-ft steel beam to imbed on a poured concrete wall. The beam was hung by two clips, one on either end. Employee #1 was sitting on top of the wall, tied off to the beamer, which was attached to the beam. The clips failed and the beam fell, pulling him off the wall. He fell 15 feet, striking his head on the concrete. Employee #1 suffered massive head trauma and was hospitalized on life support. He was taken off of life support on August 26, 2010 and died the next day.
