Employee Is Crushed and Killed by Falling Pallet

On August 27, 2010, Employee #1 was operating a stand up forklift to retrieve frozen product from a pushback rack when a pallet of frozen product containing 30 cases and weighing 1440 pounds struck him. At approximately 9:30 p.m., coworkers found Employee #1 on the floor behind the lift covered with the cases of frozen product. The forks of the lift were in the N aisle at location 117 and on the forth level the rack. Emergency medical personnel were summoned and transported him to a hospital where he was pronounced dead.
