Employee Fractures Arm in Fall from Ladder

On June 24, 2010, Employee #1 was making preparations to access the roof of a residential dwelling. After laying a plastic tarp on a concrete driveway and placing an extension ladder on the tarp with its top leaning against the garage roof, he began to ascend the ladder. The point from the top of the ladder to the ground measured approximately 9 feet. As Employee #1 reached the fourth rung of the ladder, it began to slide and then fell. Employee #1 attempted to break his fall by grabbing the garage gutter, but he was unsuccessful. Employee #1 landed on his left forearm, fracturing it in several places. Emergency services were contacted and, upon their arrival, Employee #1 was taken to Mt. Carmel West Hospital, where he underwent surgery to repair the broken bone.
