Investigation Summary
At approximately 11:00 p.m. on April 30, 2010, Employee #1 was performing a sanitation quality assurance inspection of the Reitz cheese processor. She had been employed with the company since August 29, 2008. Employee #1 was performing an inspection for sanitation after a coworker had sanitized the machine. Employee #1 spotted some minute food debris on a blade and attempted to scrape it off with left thumb nail when the blade unexpectedly rotated, amputating Employee #1's left thumb. The equipment was not operating at the time and was not locked out. All guards were in place, but were not adequate to prevent Employee #1 from contact with the point of operation.
