Investigation Summary
On July 13, 2010, an employee was inspecting a waterline in preparation for repair, and he stepped into a washout and fell into a hole that was approximately 3.5-feet deep by 4-ft in diameter. The washout was filled with hot water of approximately 140 degrees. The employee was able to remove himself from the hole and call for aid. The employee died in the hospital 10 days later from burns sustained during the incident.
