Worker Amputates Four Fingers on Ungarded Power Press

On March 19, 2012, Employee #1, a 27-year-old male with AFC Finishing Systems Inc, was operating an unguarded power press without any additional safety devices in place when he inadvertently actuated the press with an unguarded foot pedal. Employee #1 had four fingers on the right hand within the point of operation when the press cycled and sustained amputations to each of the four fingers. Employee #1 was driven to the local hospital by the company foreman where he was admitted for three days after undergoing surgical procedures on each of the fingers involved. Employee #1 was hurt on the first day of his employment with this employer and within approximately two-hours of starting work.
