Investigation Summary
On November 23, 2011, Employee #1 was working as a compression roller mill operator for John Boyd Enterprises, Inc., dba, JB Radiator Specialties, based in Sacramento. Employee #1 had finished running parts and needed to clean compression rollers that were located towards the center of the mill. The guard was removed and the mill turned on to allow Employee #1 to use a rag to clean the turning rollers. As she was cleaning the last set of rollers, the rag and her hand contacted a moving part. After Employee #1 pulled her gloved hand free, she discovered that the tip of one finger had been amputated. Employee #1 was transported to Methodist Hospital in Sacramento, where she underwent surgery that included the use of a skin graft, to address the finger injury. Employee #1 was released from the hospital the following morning.
