Employee's Fingers Are Amputated While Cleaning Tank

At approximately 5:30 p.m. on July 15, 2011, Employee #1, a brew master at a full-service restaurant and brewery, was cleaning one of the tanks used in the brewing process. The tank had a split grate (approximately 150 lbs.) on the bottom to filter the mash out of the beer. He raised the grate to approximately a 90-degree angle and placed the grate lifting handle in a position to brace the grate in this position while he washed out the bottom of the tank. When the handle became dislodged, the grate fell and partially amputated Employee #1's left index finger and middle fingers. He was hospitalized for treatment.
