On May 11, 2012, Employee #1 and a coworker were just finishing their shift and had been loading supplies on the work truck for the following day. The coworker left the immediate area of the truck for a few minutes and upon return, found Employee #1 lying face down on the asphalt paving with his arms tucked on his chest.
