Employee Is Struck by Falling Object and Receives Injuries

On March 31, 2011, Employee #1 was working in a warehouse. He was part of a larger crew assigned to move rice filled totes from a large group of stacked totes to waiting trucks. Employee #1 was on the ground but still attached to an overhead fall protection system via his lanyard. As a tote was being removed from a stack of four totes, the forklift driver yelled out that the stack of four totes was going to fall. Employee #1 like other employees tried to get out of the way. However, a falling tote fell on Employee #1's lanyard pulling him in the path of a falling tote that struck him on his head. He sustained fractures to back, neck, and chest. Emergency personnel were summoned. They arrived and transported him to a medical center, where he was hospitalized for two days.
