Employee's Fingertip Struck by Falling Pallet and Amputated

On January 21, 2011, Employee #1 was operating a forklift. As he attempted to move a stack of pallets, he disrupted a second higher stack of pallets located directly behind the first stack. The second stack tipped over, striking the first stack. He lifted his hands to cover his head from the falling pallets and inadvertently positioned his fingers through an opening of the forklift cab. A pallet struck the middle finger of his right hand, causing an amputation to its tip. He was transported to a medical center. At the medical center, Employee #1 underwent treatment and received care and was then hospitalized.
