Investigation Summary
On November 16, 2010, Employee #1 was assisting with setting up tables for lunch in the gym adjacent to the kitchen. The tables had built-in benches that were supported by attached lower legs. While setting up the tables, she stepped back from the benches and her left shoe caught one of the bench legs. She fell and landed onto her left hip. Employee #1 was transported to a medical center. At the medical center, she received treatment and care for unspecified and serious injuries and was then hospitalized.
