Investigation Summary
On December 9, 2012, Employee #1, a driver, was hauling logs in a tractor-trailer. He stopped his truck at a checkpoint to drop off paperwork about the vehicle load. While the truck was in a park position, there were no vehicle blocks behind or in front of its tires or were its brakes fully activated. Employee #1 stood outside the vehicle cab, when the truck rolled, striking and killing him.
