Employee Dies of Asphyxia in Confined Space

On August 3, 2011 Employee #1, a manager at a KOA Campground, and the owner/sole proprietor of Tri-State Septic Service, entered a 10,000 gal septic tank that was in use but needed repair. While in the tank they both died of asphyxia from gases present in the tank or a lack of oxygen.
