Investigation Summary
On June 02, 2010, Employee #1 was engaged in the setting of a structural steel beam during residential construction. Employee #1 was apparently attempting to place one end of the steel beam into a support pocket located on the top portion of the concrete basement wall, when it slipped and fell to the basement floor. Employee #1 either fell on top of, or was struck by the steel beam. Employee #1 died at the scene of the accident and was later taken to the Medical Examiners office.
