Investigation Summary
Employee #1, was a member of a crew, removing a standing seam metal roof and the insulation blanket below it, from a steel frame warehouse building. Employee #1 unhooked himself from the life line, possibly to untangle it from electrical cords, and fell over the retreating edge of the roof to the concrete floor approximately 30 ft below. Employee #1 died at the hospital from his injuries.
