Investigation Summary
On May 4, 2010, a contracted service technician from an outside vendor was training employees from the maintenance and production departments on how to use and operate an industrial food wrapping machine. The machine had been recently sent offsite for a rebuild/retrofit. After product fill bags became jammed in the product fill port, an employee applied the STOP mechanism on the side of the machine. Another employee, Employee #1, reached through the machine guard to try and clear the jam. Employee #1 was not aware that the machine was designed to complete its cycle, when the STOP button was activated. His right index finger was partially amputated, when it entered the jaws that contained a knife to cut filled bags.
