Investigation Summary
An employee was feeding a piece of flat metal stock into a pyramid calendar roll, when the piece started to go in crooked. When the employee hit the bottom of the piece with his hand, his gloved hand became wedged between the piece and the roll. The roll pulled his hand into the machine, and he sustained a partial amputation to two fingers and a crushed hand.
